- Release of Information for records that would be helpful to inform the diagnostic interview (e.g. psychological evaluation, psychiatrist, school IEP, etc)
To be filled out at your first appointment(or can be completed and brought to the appointment):
- Releases of Information for anyone you want us to coordinate with (e.g. step-parents, school staff, doctors, case managers, etc)
